Have you arranged an ACAT assessment by contacting My Aged Care on 1800 200 422* or visit myagedcare.gov.au to find your local team?(Required)

Have you estimated your fees and charges?(Required)

Have you submitted an application to the home or homes of your choice (just ask for the aged care home for the form) and fill out a Residential Aged Care Combined Assets and Income Assessment form and submit it to Centrelink or DVA?(Required)
